ADOPT-A-SCHOOL
APPLICATION QUESTIONNAIRE
*Schools
entering the second year of their adoption do not need to complete this
questionnaire.
All
schools wishing to be considered for the Adopt-A-School program should complete
the following questionnaire. This
questionnaire is required of schools who are new to the program, as well as
those who have completed a two-year adoption with an arts organization, and
would like to continue in the program with another artist or arts organization. All schools must include a signed copy of
the Adopt-A-School Application to be considered for the next school year.
Please answer the following questions
completely. Your responses will be
evaluated by a panel of artists and teachers to determine your school’s
potential for success with Adopt-A-School.
It is helpful to submit typed responses on a separate sheet(s) of
paper. Applications are due to the Art
Links office by April 30. Your school will be notified of your
application status by May, 31.
1)
Explain
any programs or activities your school is currently implementing that involves:
a) Integrated learning
across subject areas
b) Standards as a basis for planning and
evaluation.
2)
List
three reasons why your school would be a successful partner with an arts
organization.
3)
What
arts experiences have the students in your school had over the past three years
that would help prepare them for a program like Adopt-A-School?
4)
Describe
your Adopt-A-School team in terms of grade levels, subject areas, teaming and
strengths.
5)
Write
an example of a lesson or unit plan that involves: two standards from your
district’s approved curriculum, two different subject areas, a
related arts activity. Choose the grade
level this lesson would target, and include that in the plan as well.
